Do prostration three times to Gurus and the Triple Gem, then fold palms together and sincerely recite:

1. Namo Sakyamuni Buddha (Three times)

2. Lineage Refuge Tree Supplication (Three times)

Original Purity, oh, Patriarch Guru Chen!
Vajra Lotus, oh, Guru Lin!
The Right Dharma lineage of Manjusri and Guan Yin,
Harmonized wisdom and compassion to spread everywhere!

I take refuge in Vajra Guru, Buddha, Dharma and Sangha. (Three times)
Namo Guru, Yidam, Dakini, Protector. (Three times)

3. Fold palms together and sincerely recite:

In a blink the swift changes of impermanence may arrive,
Sentient beings are drifting in the ocean of suffering,
This altruistic session born of sober clarity is all too precious,
For all beings this sadhana is practiced to realize Bodhi.

4. Original Purity

Rest the hands in Dhyana mudra, and silently recite three times:

All things are conditional phenomena,
Mutually dependent and coexistent;
Renouncing antagonistic discriminations,
All appear as original purity in oneness.

Visualize the whole Dharmadhatu return to original purity, and appear as boundless sky-blue light.
From this boundless brightness the Ten Realms of Dharmadhatu appear like rainbow, vivid but insubstantial.

5. Taking Refuge

Fold palms together and sincerely recite three times the following refuge formula while visualizing that all sentient beings are gazing with admiration at all holy beings in the sky and recite together in unison:

Taking refuge in Guru, merge into Dharma lineage of Wisdom Life,
everlasting reliance in dependence.
Taking refuge in Buddha, return to limitless oneness,
everlasting reliance in abiding.
Taking refuge in Dharma, resume original purity in intention and action, everlasting reliance in concord.
Taking refuge in Sangha, put into deeds activities of the enlightening path, everlasting reliance in observance.

6. Four Boundless Minds

May all beings have happiness and its causes!
May all beings be free from suffering and its causes!
May all beings be inseparable from the joy that is free from suffering!
May all beings be distant from discrimination and preference, and abide in universal renunciation!

7. Offering

Visualize that all sentient beings take delight in participating the making of extensive and long-term offerings and donations, and that they join the practitioner in reciting in unison the following stanza:

Filling the whole Dharmadhatu with treasures,
Pouring all attainments of the wisdom path,
Offered without reluctance nor omission;
Beg to accept out of compassion and empathy,
Exhibit skillful display of the wondrous Dharma,
Salvage without neglect nor omission.

8. Praise

Regarding Guru as no different from Buddha,
In original oneness all distinctions fade away.
Mind without grasping accepts things as such,
Spreading Dharma to help all without waning.

Tantric teachings of Guru Rinpoche rely on Your Reverence to pass on,
Wisdom lamp of Manjusri rely on Your Reverence to continue shinning,
Marrow of Tang Tantras rely on Your Reverence to reveal and explicate,
Please stay long in this world to benefit all with your salvation activities!

9. Chanting Heart Mantra of Guru and Visualization Heart Mantra of Guru Lin:

Weng A Hong Ban Zha Gu Lu Ei Wang Die Dong Xi Di Hong

Visualize oneself in one's ordinary or Yidam form and Guru Lin as Golden Guan Yin appears in the space in front. While repeating Guru's mantra, visualize Guru bestows the four empowerments successively as follows:

a) The white syllable "Om" in Guru's crown chakra emits white light ray into the practitioner's crown chakra. This is the first empowerment.
b) The red syllable "Ah" in Guru's throat chakra emits red light ray into the practitioner's throat chakra. This is the second empowerment.
c) The blue syllable "Hung" in Guru's heart chakra emits blue light ray into the practitioner's heart chakra. This is the third empowerment.
d) A second blue "Hung" emerges from the blue "Hung" in Guru's heart chakra and then flies into the heart of the practitioner and melts into oneness with the practitioner's essence. This is the fourth empowerment.

10. Hundred Syllable Mantra (Three times)

Om Ban Dsa Sa Do Sa Ma Ya Ma Nu Pa La Ya Ban Dsa Sa Do Deh Noh Pa Di Tsa Dri Dso Mi Pa Wa Su Do Keh-yu Mi Pa Wa Su Po Ke-yu Mi Pa Wa Ah Nu La Do Mi Pa Wa Sa Wa Si Di Mi Dsa Ya Ta Sa Wa Ka Ma Su Tsa Mi Chi Dam Si Yang Ku Lu Hung Ha Ha Ha Ha Ho Pa Ga Wen Sa Wa Da Ta Ga Da Ban Dsa Ma Mi Mu Tsa Ban Dsa Pa Wa Ma Ha Sa Ma Ya Sa Do Ah

11. Dedication of Merits

1. May virtuous gurus remain with us and those departed return soon!
2. May perverse views and violence soon become extinct and Dharma spread without hindrance!
3. May all beings proceed diligently on the path and achieve Buddhahood before death!
4. May all beings develop Great Compassion and never regress until they reach perfect Buddhahood!
5. May all beings develop Great Wisdom and never regress until they reach perfect Buddhahood!

May all beings attain enlightenment soon!
